Repeat Prescriptions...

For those patients who are on regular medication and wish a further supply, please let us know at least THREE WORKING DAYS in advance.

You have the following options available to you:

• Serial Prescriptions Service - serial prescription service means that you can get your regular, repeat medicines straight from your pharmacy without having to order a prescription from your GP beforehand. Click for more information
• Download and print repeat prescription request form, fill it in and return to the practice.
• Complete a request form (the tear-off page opposite your last prescription) and either put it in the box at the front door in Armadale or in the box at the front door in Blackridge.
• You may also request your chemist to order and collect the prescription for you.
• Fill out the On-line Patient Services Repeat Prescription Request form. Login here
